Dr William Portuese will like to learn about your goals and expectations at your initial meeting. He wants to get to know you as a whole person. He will utilize a state-of-the-art imaging tool to show you how your nose will look after surgery after taking some time to get to know you. Patients frequently have no idea how much rhinoplasty can alter their profile. The ability to see this is a significant tool that can aid in the deciphering of your objectives.
A deviated septum is commonly caused by a broken nose. Patients with a deviated septum are usually advised to try medicinal treatment first before seeking surgical treatment for nasal blockage. If allergies are discovered, medical treatment may include cortisone nasal sprays, antihistamines, decongestants, allergy testing, allergy desensitization needles, and environmental avoidance. After medical treatment has failed, a septoplasty may be considered to enhance airflow dynamics through the nose. An ENT doctor performing a comprehensive examination of the front and back portions of the nose must document the medical requirement at the time of inspection and consultation. A CAT scan of the sinuses will also reveal whether or not the septum is deviated. Other causes of nasal obstruction, such as recurrent sinus infections, turbinate hypertrophy, valve collapse, and vestibular stenosis, are not caused by a deviated septum. Each of these problems is dealt with in a unique way, either through medicine or surgery
Dr. William Potuese, a top ENT specialist, treats this irregularity with the most advanced cartilage-sparing procedures, allowing him to fix the deviated septum while preserving your cartilage in case you wish to have a rhinoplasty later.
After medicinal therapy has failed, a septoplasty procedure is performed as a medical necessity. Patients were put to sleep in our Medicare-certified outpatient surgery clinic under a general anesthetic by a board-certified anesthesiologist. A minor incision is made on the inside of the nose after the patient is put under general anesthesia. It is around 1/2 inches long. The skin along the septum on the inside of the nose is raised, and the deviated, obstructive cartilage and bone is removed
To close the incision, 2-3 dissolvable stitches are put in the septum once it has been straightened. In our practice, no packing or stents are used in the nose. The septoplasty surgery is not painful because no packing is placed within the nose. For at least one week, patients have significant congestion and are unable to breathe out of their nose. Because the surgery is done entirely on the inside of the nose, there is no obvious bruising on the outside. Patients must avoid strenuous exercise and blood thinners for two weeks following surgery. It takes about a month for the body to fully recuperate. Patients who have increased airflow through the nose report that their snoring has improved, that they can sleep better at night with their mouth closed, and that they no longer have dry mouth when they wake up. Their sense of smell has improved, and they feel more energized.

In addition, depending on where the skin and cartilage are removed, the tip of the nose can be turned upwards or downwards. When there are still large nostrils after a primary rhinoplasty, an alar-plasty is performed. To narrow the nostrils, an alar-plasty involves removing a wedge of skin from the base of the nostrils. Depending on the surgeon’s preference, revision rhinoplasty can be performed using an open or closed rhinoplasty method. All of the incisions for a closed rhinoplasty are made on the inside of the nose to have access to the tip, bridge, and nasal bones to perform the necessary adjustments. The same incisions on the inside of the nose are used as well as an external incision across the columella in an open rhinoplasty.

Because septoplasty is an outpatient treatment, you will be able to go home and begin the healing process the same day. You should expect some pain and swelling in the area for the first few days, but it will be bearable if you elevate your head, avoid irritating your nose, and take your prescribed pain medicine as indicated.
Breathing Difficulties Out of the Nose (Nasal Obstruction)
To avoid difficulties, you will also be recommended to refrain from engaging in intense activities and blowing your nose. Dr. Portuese will give you with specific counseling during your post-operative appointments, but you should be able to return to work and your normal daily routine within one week.
